そうですね。
a2.rb で －I. を使用する場合だと、require が失敗するだけで、
いきなり終了する現象は出ません。
require が失敗するケースも対策されるとうれしいです。

仕事で使用している Windows 7 64bit の環境で試しましたが、現象発生します。
こちらで確認している条件は次の2点です。
* カレントディレクトに日本語を含む
* RUBYLIB に . 設定されている
ログを貼ります（プロンプトが2行になっています）が、どうでしょうか。

puts 'a'
require'./b'

Kernel#exec(cmdname, arg1, ...) 形式でプログラムを呼び出す際、argが日本語を含んでいると
化けた文字列が渡されます。
同じ内容を Kernel#excec(cmdline) 形式で呼び出すと問題は発生しません。
直接呼び出す場合は問題なし

String#casecmp dose not return nil but raise TypError for incomparable argument.
So

Preview はRuby本体だけで、zlibなどは別途用意する事と理解しました。

gem をインストールしようとするとエラーになります。
zlib が足りないようですが、対処方法がわかりませんでした。
preview はやはり敷居が高いですね。

09:49 AM Ruby Bug #12748: irb の評価結果が2回表示されます

io.c の io_fwrite() に問題があるようです。
この関数内で #ifdef _WIN32 内の、rb_w32_write_console() と、その下の io_binwrite() の両方が呼び出されるために、2回出力されています。
リダイレクトすると片方だけになります。
