Backport #6004: backport r32527, r32567 (method_added should not called for undef_method)

r32527, r32567 (undef_method した時に method_added フックが呼ばれてしまうのを修正 & テスト追加)は不具合修正だと思うのでバックポートが必要ではないかと思います。 [Bug #5015]
